How do I obtain a person with disability placard?

To obtain a parking placard, you will need to submit Form MV-145A ‘Person with Disability Parking Placard Application.’ This form will need to be certified by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio). A police officer can also certify the request but only when the applicant does not have full use of a leg or both legs, or is legally blind. NOTE: Health Care Providers may only certify disabilities within their scope of practice.

To obtain a disability license plate, you will need to submit Form MV-145 ‘Application for Person with Disability, Hearing Impaired, Disabled Veteran, or Severely Disabled Veteran Registration Plate or Person with Disability or Severely Disabled Veteran Motorcycle Plate Decal' (Form MV-145). Like the 'Person with Disability Parking Placard Application' (Form MV-145A), this form will need to be completed by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio) or a police officer. Requests for duplicate registration credentials (license plate, renewal sticker, or registration (owner’s) card) can be made on the 'Application for Duplicate Registration Card, Replacement of Registration Plate, Renewal Sticker or Weight Class Sticker’ (Form MV-44). Notarization is required only if you are requesting a free replacement of a previously issued product, which was not received within a 90 day period from the date of issuance.
